This privacy policy sets out the basis on which any personal information we collect from you, or is provided to us by you or will be processed by us. Please ensure that you understand the policy by reading the following information.
INFORMATION WE COLLECT FROM YOU
Enquiries
We may record your details to track and follow-up on enquiries and schedule appointments. This could include information such as your name, email address or phone number. This allows us to supply you with information you need about our professional services, on the basis that it is necessary for to provide information and a quotation for our services. Failing to provide us with information for marketing purposes has no bearing upon the provision of your treatment which is guided by our medical and ethical principles.
Medical Records
Information must be collected as part of your registration with the clinic. This includes data such as your name, address, date of birth, email address, phone number, next of kin and medical history. This is to perform the contract that we have with you, for internal recordkeeping, billing and accounting, and to respond to any queries, complaints or requests for further information. The basis on which we do so is that it is necessary for our performance of the contract we have with you or is necessary for our legitimate interests in managing our business and improving our professional services, and to comply with our regulatory obligations.
Failing to provide us with this information will result in us being unable to provide treatment.
Keeping in touch
We hope to be able to use the details maintained securely on our database to inform our customers of the services that we provide and develop on the basis that it is necessary for our legitimate interests in promoting our professional services. If you decide you do not want to receive such information, you can unsubscribe using the link in the email that we send you. Future marketing communications will also contain a simple way to opt-out of receiving any further marketing communications from us.
We automatically collect information with each visit to our website www.infinityclinic.co.uk to analyse the use of the website and fix problems that might develop. Such information includes the following:

WHO WE SHARE YOUR PERSONAL DATA WITH
Infinity Aesthetics considers our obligations under the General Data Protection Regulations (GDPR) and our clinical confidentiality requirements to be fundamental in managing data and confidentiality.
Sensitive information relating to your medical history will be kept confidential and will only be disclosed to the individuals involved with delivering your treatment.

Other members of Infinity Aesthetics staff as part of your ongoing care provisions or to analyse the business
Occasionally we may have to share limited data with professional third parties such as pharmacies to provide prescriptions or if we needed to refer onwards to colleagues who are more experienced in a particular form of treatment that is deemed more suitable for you (with your specific consent in these cases). Such third parties would be under obligation not to share data onwards.
Rarely there would be a requirement under a legal duty to disclose your personal information or share your personal information in order to comply with any legal or regulatory requirement, obligation or request. This includes the police for the prevention or investigation of a crime, our Insurers, legal advisors, or other third parties who need access to it to manage, investigate, or defend claims or complaints.
YOUR INFORMATION
We cannot guarantee that the information such as your basic details and questions you send us over the website is secure (although the data submitted through the Jotform link with more sensitive information is encrypted and deemed secure), but once we receive it we will take all reasonable steps to protect the information you supply to us.
Infinity Aesthetics will not transfer your data outside of the United Kingdom and Ireland. Data is stored electronically in a cloud-based system with a data processing contract and robust physical and data security controls in place.
If you contact us with an enquiry but do not subsequently become a patient, it is our policy to keep your data unless you instruct us not to.
If you are or become a patient, we will retain contact information (including personal data) indefinitely.
Personal data relating to our professional contacts will be retained for 5 years, or until you indicate otherwise to us, but we will aim to update our contacts’ preferences periodically.
We can delete any personal information on request, or supply a copy of any information held to you on request.
COOKIE POLICY
Our site uses cookies to distinguish you from other users of our site, which helps us provide our customers with a good experience on browsing our website and allows us to improve the site.

Occasionally our website might contain links to third parties. If you were to click on these links, please note that other websites have their own privacy policies and that we do not accept any responsibility or liability for these policies. Please check these policies before you submit any personal data to these websites.
In some circumstances, you have the right to be forgotten and we will erase all data held about you. Medical records are exempt from erasure. Requests for erasure should be made in writing to hello@infinityclinic.co.uk and will be assessed on a case by case basis.
ACCESS TO INFORMATION
The General Data Protection Regulation gives you the right to access information held about you. Any requests should be made in writing to Dr Owen Thomas at hello@infinityclinic.co.uk
All records will be supplied within one month of receipt of the Subject Access Request.
All such requests will be subject to identification checks. Although we do not charge for the first copy of your record, we reserve the right to charge a small administrative fee for all subsequent copies of the same record.
